As part of your SPARK Premium membership, Members under the age of 65, are eligible for $10,000 of Basic AD&D coverage at no cost to you, and all SPARK members may apply for $500,000 in optional AD&D coverage. Acceptance is guaranteed—there are no medical exams or questions. To request coverage, simply sign up using the online web request form.

Optional AD&D Options

Insure yourself as well as your dependents with Accidental Death & Dismemberment Insurance (AD&D). AD&D provides benefits for losses caused by accidental bodily injury.

Members can add $500,000 of AD&D coverage and also add $250,000 coverage on your spouse. Eligible dependent children can be covered for $10,000 per child. A newborn child is not your qualified dependent until the first premium due date following birth.

Who Is Eligible To Apply?


  • Premium Members of SPARK who are less than 65 may apply for the Basic AD&D.
  • All Members of SPARK who are less than age 65 may apply for $500,000 of the Optional AD&D coverage.

Eligible Dependents: Spouse/domestic partner and dependent children less than 26 years old. Your children include your legally adopted children, step-children and foster children who depend upon you for support and maintenance.

The term includes: (1) a shuttle bus, tram, Limousine, or other vehicle used to transport people within an airport; and (2) chartered aircraft. But it does not include any aircraft: owned; operated; controlled; or leased by or on behalf of the Contract Holder or any of its subsidiaries or affiliates or its customers.

This type of plan is NOT considered “minimum essential coverage” under the Affordable Care Act and therefore does NOT satisfy the individual mandate that you have health insurance coverage.
